Low-Cost Spay/Neuter Surgery Appointment Request Form

To request an appointment to have your pet spayed/neutered through the Low-Cost Friday and Saturday Spay/Neuter program, held at the ATLANTA, GA location, please complete the information requested below. Please be sure to fill out this form in its entirety. Failure to do so may result in delayed processing of your appointment request.

Please do not submit more than one appointment request per animal. Note that more than one appointment date may be assigned for multiple pets.

Pets must be under 6 years of age to be eligible for service through the Low-Cost Saturday Spay/Neuter program. If it is determined that the pet is over 6 years of age at the time of his/her appointment, onsite staff will refuse service.

Microchip Information

A microchip is a small chip inserted just under the skin between your pet's shoulder blades that can be scanned by a local shelter/veterinarian in the event that your pet is lost/runs off and is found. There is no addtional cost for microchipping your pet during surgery.
